Advance Directives

Planning for possible future incapacity by providing your loved ones and caregivers with the authority they need to make medical and financial decisions for you is truly a gift. Preparing advance directives can spare them the emotional anguish and conflict that often comes with being unaware of a loved one’s directives and even more importantly, the authority to make such decisions on their behalf.

A Health Care Proxy allows you to designate an Agent who will make health care decisions on your behalf should you become incapacitated.  A Health Care Decisions Declaration (aka Living Will) gives you an opportunity to set forth your wishes with regard to your care and comfort should your doctor determine that you have a terminal condition.

The Durable Power of Attorney gives your Agent broad powers to handle matters pertaining to financial affairs, during your lifetime, in the event you are unavailable or unable to do so. This financial authority survives any future disability or incompetency.
